The investigation of accidents or incidents in the fields of Mental Retardation or Mental Health is not an easy task. In the best of circumstances the investigation faces damaged prides and in the worst cast there can be significant loss of life and program integrity and federal financial participation.

Because of the paucity of trained executive level investigators in the country, state agency heads have in the past resorted to using various consultants, who operate out of their homes in many cases, to investigate major events that can shake the foundation state government.

The mere fact that an individual has obtained a degree in an area such as nursing or has "worked" administratively  in the field of human services does not qualify that consultant to provide high profile incident investigations. May court rooms have echoed the silence of those consultants when asked if they have had any formal training in the investigation sciences.

Even more surprisingly there are cases where consulting contracts to review deaths in high profile institutional care systems are given to individuals who have never done an incident investigation or undergone any training in the field.

Rather than complicating the incident by utilizing a "street cart" consultant and possibly expanding the potential  incident liability to the agency (not to mention press coverage) this website provides department heads the resources to match the expertise of the investigator with the level of concern.

Our executive staff have decades of experience dealing with high profile and high risk incidents. Depending on the particular aspects of the incident we are able to extend our central capabilities with national expert staff who have provided such case specific services to benefit the agency.

Best Practices in Incident Investigations

Current Challenges to Effective Incident Review Management

Internal review teams seldom provide safe outcomes since they have many traditional problems to deal with such as:

bulletPractical realities of administrative and clinical care issues;
bulletDistinguishing between individual responsibility and systemic problems;
bulletExisting facility cultural history;
bulletUnclear or inadequate documentation; and
bulletHaving no ability to contribute assessment and intervention strategies that had not been formerly accessible in the past.

How you will benefit by working with our organization?

We provide the professionalism, credibility and best practices of the investigative sciences in the MR/DD/MN field and make them available for you so that your agency can:

bulletDevelop effective controls with our incident review program;
bulletHave the ability to decide the risks of recurrence;
bulleteliminate the blame fixing and finger pointing;
bulletIdentify emerging trends; and
bulletimproved employee relations

Risks must be well understood, and risk management approaches developed, before decision authorities can authorize a program to proceed into the next phase of service.

Often in house reviews fail to provide comprehensive information to decision authorities for a number of reasons including:

bulletObtaining limited information during high profile incident investigations;
bulletNot performing a Root Cause Analysis;
bulletUsing a limited variety of search strategies;
bulletInexperience in "how to write an investigation report"; and
bulletHaving enough credentialed staff to create the appropriate constitution of the incident team

Types of Incidents Investigated

Our staff provide all kinds of emergent and on-going incident reviews as required. Some of the more common investigations include but are not limited to:

bulletDeaths due to a variety of causes, including forensic investigations;
bulletA suicide attempt by an individual;
bulletAn outbreak of a serious communicable disease;
bulletAn incident requiring the services of a fire department or law enforcement agency; and
bulletInjury, trauma or illness requiring inpatient hospitalization, that occurs while the individual is at the facility or under the supervision of the facility
